Information for New Students



 Health and Immunization Compliance


The Johns Hopkins University Student Health and Wellness Center requires that all incoming students be compliant with these health and immunization requirements.


Students are required to provide proof of immunization against measles, mumps and rubella as well as, receive screening for Tuberculosis (if applicable). All incoming students must submit a pre-entrance health form signed by your health care provider and complete three online forms via the Health WebPortal for verification purposes. 


Important: Submission of your health and immunization information requires completion of 3 steps. Please follow the instructions carefully and in the order outlined in the image below to ensure your records are processed without delay. More detailed instructions for compliance have been provided below the image.


Failure to comply with the pre-entrance health requirements prior to your arrival on campus will block you from adding or dropping classes and interrupt orientation which will result in a $100 health form completion fee.


 Religious Exemption Request: All religious exemptions/accommodations must go through the Office of Institutional Equity and submit this form.

Attention Athletes: you are still required to complete  the SHWC pre-entrance health forms and comply with all requirements even though you have submitted health related information to the athletics department.

Post-doctoral fellows: 2014 Pre-Entrance Health Form


Step 1: Have your health care provider complete and sign the pre-entrance health form

  • No physical examination is required!
  • Health forms must be completed in English
  • Health forms must be signed by your health care provider (in lieu of a physician's signature you may attach a copy of your official immunization record to your health form).
  • Please do not submit any CD's or actual laboratory films
  • Make a copy of your completed pre-entrance health form for your records.
  • Do not submit pre-entrance health form until you have completed step 2




Step 2: Register for the Pyramed Health WebPortal and Complete the three online pre-entrance health forms

  • Once you have received your completed health form from your health care provider, register for the Pyramed Health WebPortal. 
  • You will need your 6-character Hopkins ID, found in ISIS*, which can be alpha, numeric, or a combination of both.

*(Post-doctoral students please email to obtain access to the Pyramed Health WebPortal.)

Register and complete online forms:

1) Go to
2) Select "Click here to register"
3) Register using your Hopkins ID, Hopkins email address, and date of birth
4) A temporary password will be sent to your Hopkins email address
5) Return to the login screen
6) Enter your Hopkins ID and the temporary password
7) Go to the my profile tab and select "change password"
8) Change your temporary password to a permanent password that you can remember
9) Go to the my forms tab
10) Complete the three pre-entrance health forms (Health History, Immunization and Consent to Medical Procedure)




Step 3: Submit your health form

  • Once you have completed the three online health forms, submit your signed pre-entrance health form to the Student Health and Wellness Center using one of the acceptable ways for submission.
  • IMPORTANT: Submitting your signed pre-entrance health form to our office prior to completing the online health forms via the Health WebPortal will significantly delay processing.
  • Allow 5 business days for your records to be received, reviewed and processed after submitting!




Acceptable ways to submit your health form:


There are currently only three acceptable ways to submit your completed health form.

  • Fax to the Student Health and Wellness Center at (410) 516-4784 ; please include a cover sheet that contains your full name, school and date of birth. 
  • Email: (please submit a scanned PDF attachment of your health form)

  • Mail: We cannot guarantee the arrival of mailed forms
